We have made the following health forms available online to ease the transition for our new patients. You may print and then complete the form and bring them with you on your first visit, mail them in, or even fax the information to our office (317-846-1742). You have the option of filling the Microsoft Word versions on your computer, and then attaching them to an email to our office: newpatients@kristoffdds.com.
